A Nurse Angels Home Health, Inc. is a licensed Home Health Agency which provides coordinated and comprehensive home health services to homebound patients in the comfort of their own residences. All services are under the direction of your physician and supervised by a Registered Nurse or Licensed Therapist.
Our agency offers non-medical and skilled home health services with a comprehensive assessment designed to improve your quality of life. Our dedicated health care team consists of Registered Nurses, LVN’s, PT’s, OT’s ST’s, Medical Social Workers and Home Health Aides.
Admission Criteria:
- A Physicians referral is required to receive our skilled home health services
- Must be homebound
- Must have short-term health needs
- Must be under the care of your Physician
- Must require additional skilled care services following discharge from hospital or other healthcare institution
